Key Responsibilities: • Offer comprehensive care within the RN scope of practice • Engage in client education and group health promotion • Provide telephone triage and follow-up support • Conduct outreach and represent the centre in community initiatives • Participate in quality audits and team evaluations

Requirements: • RN registered in valuable standing with the College of Ontario • Minimum three years nursing experience in primary care • Bachelor’s degree in nursing required • Strong assessment, promotion, and counseling skills • Experience with chronic disease management is a plus
