This role oversees key initiatives including the Family Transition Program, Adult Day Program, and transitional services for admissions, ensuring alignment with the home’s mission, values, and person-centered care philosophy to enhance quality of life and inclusion.QUALIFICATIONS:Minimum five (5) years of experience in gerontology, long-term care, or community program delivery.Demonstrated experience in staff supervision, program coordination, and leadership.Knowledge of applicable legislation, including the Fixing Long-Term Care Act, Resident Bill of Rights, and best practices in person-centered care.Proven ability to build and maintain partnerships with internal and external stakeholders.Robust communication, interpersonal, analytical,
