Take part in shaping the future of youth health as a Nurse Practitioner with the Durham Community Health Centre’s Youth Wellness Hub. This permanent part time role focuses on addressing complex health needs of young people.
As a Nurse Practitioner, you will deliver youth-centered primary care, addressing key issues such as mental health and substance use. You’ll engage with a multidisciplinary team to ensure comprehensive care delivery, improving health equity and community wellbeing across various locations in the Durham Region.
Key Responsibilities:
• Provide primary care and wellness services tailored for youth • Manage pharmacotherapy within your NP scope of practice • Facilitate care coordination and intake assessments • Engage youth in their health decisions and promote inclusivity • Support continuous improvement and adherence to best practices
Requirements: • Master’s in Nursing with Primary Care NP certification • Active CNO registration as an NP in Ontario • Minimum 3 years in primary care, preferably with youth • Experience in integrated care settings is a plus • Strong documentation and communication abilities
