Full Time

Salary Range: $41.15 - $58.98 /hour

St. Joseph’s Health Care is willing to consider applicants who are eligible for Community Commitment Program for Nurses (CCPN). This program provides $25,000 in grant funding to eligible RPNs/RNs/ Nurse Practitioners in exchange for a two-year full time commitment. Using the principles of client-centered care within the context of an interdisciplinary team approach, the successful candidate will provide nursing care to adult clients between the ages of 18 to 65 years with a dual diagnosis (moderate to profound developmental disability and a mental health need or extreme behavioural challenge). This role will be responsible for completing comprehensive assessments in both the hospital and clients’ home environments to support care planning, treatment, and transition needs. The role also requires collaboration and coordination with community partners and external agencies, including Regional Support Associates, Developmental Services Ontario, the Ministry of Children, Community and Social Services, group homes, community service providers, and other agencies involved in the client’s care and transition planning.

Essential Qualifications

- Current certificate of competence with the College of Nurses of Ontario
- Valid CPR certificate




- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
- Well-developed assessment, consultative practice and evaluation skills in the care of clients with severe and persistent mental illness
- Excellent working knowledge of nursing principles, theory and legislation and understanding of the nursing care needs of adults with severe and persistent mental illness
- Must be capable of establishing positive interpersonal relationships and able to work as an effective team member
- Demonstrated ability to apply and document the nursing process
- Ability to relate therapeutically with clients
- Demonstrated leadership in the initiation, coordination of, and participation in team/family conferences and clinical activities
- Excellent communication and interpersonal skills
- Excellent organizational, critical thinking and problem-solving skills
- Experience in the development of teaching materials and the teaching of clients, family and caregivers
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA

Preferred Qualifications

- Proficiency in French would be an asset.
- Psychiatric mental health nursing certificate
- Experience working with individuals with a dual diagnosis and/or in mental health

Immunization Requirements

- Provide vaccination records or proof of immunity against measles, mumps, rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
- Provide documentation of the Tuberculosis skin testing
