Key Responsibilities:
• Conduct in-depth psycho-social and family assessments • Provide individual and family counseling and support • Advocate for families regarding community services • Collaborate with the nurse coordinator for patient care continuity • Maintain clinical documentation as per standards
Requirements: • M.S.W. graduate from an accredited university program • Must be a member of the Ontario College of Social Workers • Minimum of 2 years experience in a pediatric or healthcare setting • Strong problem-solving skills and crisis intervention experience • Bilingual in English and French
