Posted : Full-Time On-site Geriatric Psychiatry Community Services of Ottawa (GPCSO) is a fully bilingual community mental health program funded by Ontario Health serving older adults in the Ottawa region. We work with older adults living in our community to optimize their mental health and/or dementia with behaviour change. At GPCSO we strive to provide excellence in mental health care for seniors.
We provide expertise and leadership in community geriatric psychiatry through holistic care to our clients and their circle of support, in collaboration with our partners.Our values are reflected in all levels of our organization, from dignity and compassion, to respecting diversity and lifelong learning. GPCSO Case Management Case managers are an integral part of a dedicated interdisciplinary team providing care to older adults. Defined benefit pension plan (when applicable) Wellness program and on-site gyms Mental health support through our Employee and Family Assistance Program Professional development and learning opportunities Reporting to the Executive Director, Case Managers are Registered Nurses, Occupational Therapists, or Social Workers. They provide assessment, treatment, and follow-up to seniors with mental health symptoms and/or dementia with responsive behaviours.
We support clients and family/caregivers to address mental health care needs through short-term case management and collaboration with other senior-focused organizations to promote independence and well‑being.
Responsibilities of a case manager: Support self-management of mental health symptoms, Enhance caregiver understanding of approaches to care that support meaningful engagement, Characterize and communicate risk and collaborate with clients, family members and health care partners to understand options, implement decisions and balance risk versus autonomy, Make referrals to community resources and other health care agencies to support client goals. Works closely with the geriatric psychiatrist in the provision of high-quality mental healthcare Collaborates with community partners to support increased capacity to care for older adults living with dementia and/or psychiatric conditions in the community. Communicates clearly and effectively with Primary Care Practitioners and referral sources Occupational Therapist with a Bachelor or Master's degree Social Worker with a Master's degree Registered Nurse Driver's license and access to a reliable automobile Bilingualism solid asset Comprehensive knowledge of psychiatric conditions affecting older adults Knowledge of community resources GPCSO promotes the principles of diversity and inclusion and adheres to the Employment Equity Act and Accessibility for Ontarians with Disabilities Act.
All GPCSO employees are required to provide proof of full vaccination for all identified communicable diseases as per our Communicable Diseases: Health Surveillance and Management policy.
