PRIMARY FUNCTION:
The Bariatric Data Clerk is responsible for the collection, abstraction, entry, validation, and maintenance of patient data for the Ontario Bariatric Network (OBN) Registry and associated databases. The role supports data quality, reporting, and regulatory requirements through accurate and timely entry of clinical information obtained from electronic medical records, clinical documentation, and registry systems.
Within this role, the employee is accountable for contributing to the delivery of Kingston Health Sciences Centre strategy and demonstrates a commitment to patient and family centred care, confidentiality, and data integrity.
PRINCIPLE RESPONSIBILITIES:
Data Collection & Abstraction:
- Reviews electronic medical records, clinic documentation, laboratory results, and related source documents to abstract required registry data elements.
- Ensures completeness, accuracy, and consistency of collected information prior to submission into registry databases.
- Identifies missing, incomplete, or inconsistent information and follows up with appropriate clinical team members as required.
Data Entry & Validation:
- Enters patient and clinical information into the Ontario Bariatric Registry databases and associated electronic systems in accordance with registry standards and timelines.
- Maintains data accuracy through ongoing validation, correction, and resolution of registry queries.
- Reviews automated edit checks, quality reports, and data discrepancies to support data integrity and completeness.
- Participates in routine data quality audits and registry reconciliation activities.
Communication & Collaboration:
- Enters patient and clinical information into the Ontario Bariatric Registry databases and associated electronic systems in accordance with registry standards and timelines.
- Maintains data accuracy through ongoing validation, correction, and resolution of registry queries.
- Reviews automated edit checks, quality reports, and data discrepancies to support data integrity and completeness.
- Participates in routine data quality audits and registry reconciliation activities.
QUALIFICATIONS:
- 2 years Office Administration Health Services diploma or equivalent education and experience.
- Demonstrated proficiency with Microsoft Office applications and computerized data entry systems.
- Ability to type 40 words per minute (will be tested)
- Demonstrated knowledge of medical terminology and clinical documentation.
- Excellent attention to detail with a high degree of accuracy in data entry and data validation.
- Solid organizational and time management skills with the ability to manage competing priorities and deadlines.
- Demonstrated ability to work independently with minimal supervision.
- Effective interpersonal and communication skills with the ability to collaborate within an interdisciplinary team environment.
- Ability to exercise sound judgment and maintain strict confidentiality.
- Demonstrated ability to adapt to changing priorities and workflow demands.
- Satisfactory criminal reference check and vulnerable sector search required.
PHYSICAL REQUIREMENTS: The applicant must be able to meet the physical demands of this position.
