Take part in shaping the future of youth health as a Nurse Practitioner with the Durham Community Health Centre’s Youth Wellness Hub. This permanent part time role focuses on addressing complex health needs of young people. As a Nurse Practitioner, you will deliver youth-centered primary care, addressing key issues such as mental health and substance use.

You’ll engage with a multidisciplinary team to ensure comprehensive care delivery, improving health equity and community wellbeing across various locations in the Durham Region. Key Responsibilities:

- Provide primary care and wellness services tailored for youth
- Manage pharmacotherapy within your NP scope of practice
- Facilitate care coordination and intake assessments




- Engage youth in their health decisions and promote inclusivity
- Support continuous improvement and adherence to best practices Requirements:
- Master’s in Nursing with Primary Care NP certification
- Active CNO registration as an NP in Ontario
- Minimum 3 years in primary care, preferably with youth
- Experience in integrated care settings is a plus
- Strong documentation and communication abilities Utilize your skills in providing culturally responsive care to enhance the health landscape for youth at DCHC.
