Responsibilities

- Conduct outreach activities to engage community members, families, and potential clients in need of addiction treatment services
- Build solid relationships with local organizations, schools, healthcare providers, and community groups to promote awareness and access to treatment programs
- Assess client needs through interviews, case management, and data collection to develop personalized care plans
- Provide education on addiction, mental health, and available treatment options tailored for diverse populations including children and pediatrics
- Support patients in navigating the healthcare system, including hospital experiences and special education resources when applicable
- Maintain accurate records of client interactions,



progress notes, and data for research and program evaluation purposes
- Collaborate with multidisciplinary teams to ensure comprehensive patient care and smooth transitions between services

Qualifications

- Experience working with addiction treatment center or clinical settings is highly desirable
- Prior hospital experience or familiarity with medical terminology enhances your ability to communicate effectively within healthcare environments
- Strong background in research methods, data collection, and case management practices
- Knowledge of addiction treatment processes, patient care protocols, and mental health considerations
- Excellent interpersonal skills with the ability to build rapport with diverse populations
- Ability to handle sensitive situations with professionalism and compassion
- Certification or training in special education or related fields is a plus
