• Clinical Support - Provide skilled nursing care within RN scope of practice - Perform tracheal suctioning as needed - Administer medications (oral, injectable, topical) - Perform wound care, dressing changes, and skin assessments - Monitor vital signs and assess changes in client condition - Support chronic disease management and symptom control - Communicate health concerns promptly to the Case Manager or healthcare team Daily Living &

• Safety Support - Assist with personal care when required (bathing, grooming, toileting) - Support secure mobility, positioning, and transfers - Reinforce safety strategies and fall‑prevention measures Documentation &

• Communication - Complete accurate nursing notes for each visit - Document observations, significant changes, and interventions - Collaborate effectively with families and the interdisciplinary team Companionship &

• Emotional Support - Build therapeutic rapport with clients - Provide reassurance, active listening,
