Position Summary:

Dan's Legacy is seeking a Social Worker to join our team in providing essential services to at-risk youth. This position requires a trauma-informed approach and a collaborative mindset, working closely with Youth Outreach Workers and Therapists to deliver support in our Counselling and Job Skills Training Programs.

This position combines professional social work, case management, community outreach, advocacy, and participant support. The successful candidate will work closely with Dan’s Legacy’s therapists, Job Skills Training Program staff, and management team to ensure youth clients receive coordinated, wrap-around support.

A significant component of this position will involve community-based outreach across the Lower Mainland, including Vancouver, New Westminster, Surrey, and other communities as required. The Social Worker will engage youth in community settings, support recruitment and engagement in Dan’s Legacy programs, connect youth to community resources, and provide practical assistance to help them stabilize and work toward their goals.

The successful candidate will be comfortable working both independently and as part of an interdisciplinary team, including working in office, community, partner-site, and outreach settings.

This Position Reports Directly to the: Clinical Counselling Manager

Location: 250-131 11th Street, New Westminster, BC, V3M 4C7 (with outreach across Metro Vancouver)

Position Type: Full-time & Fixed Term until: March 31, 2027 (with potential for extension beyond that date)

Duties & Responsibilities:

- Conduct psychosocial assessments, develop case management plans, and provide short- and longer-term support based on participant needs.
- Assist youth with housing, income assistance, food security, health care, identification, education and employment, including applications, appointments, advocacy and system navigation.
- Coordinate referrals, follow-up and transitions while promoting participant independence and self-determination.
- Complete case management plan, such as scheduling and discharging clients, and making internal/external referrals, according to ethical and professional standards as per British Columbia School of Social Workers (BCSSW).
- Utilize both Single-Session Interventions (for immediate, concrete needs such as housing applications or benefit navigation) as entry points to assess deeper barriers, and Long-Term Interventions involving comprehensive psychosocial assessments and care plans when multi-layered support is required
- Conduct regular outreach across Metro Vancouver to engage youth, connect with Dan’s Legacy programs and provide practical, low-barrier support.
- Participate in recruitment and engagement activities for counselling, Job Skills Training, the Cooking Skills Program and grocery hamper initiatives, as assigned.
- Work collaboratively with Therapists, Youth Outreach Workers,



Job Skills Training staff and Managers to provide coordinated, trauma-informed support.
- As part of the interdisciplinary team with Therapists and Youth Outreach Workers, support the delivery of the wrap-around programs, coordinate case discussions with therapists, share insights about client’s psychosocial situation and contribute to creating a comprehensive care plan in collaboration with the client, proactively coordinate services, and ensure client support (at Dan’s Legacy offices, partner sites or in the field).
- Participate in case discussions, identify barriers to program participation and coordinate supports to promote engagement and successful program completion.
- Coordinate internal and external referrals and develop relationships with community agencies, health-care providers, government services, educational institutions, and other partners.
- Represent Dan’s Legacy at community meetings and tables and advocate for participant access to appropriate resources and services.
- Assess and respond appropriately to crisis and high-risk situations, including self-harm, suicidal ideation, overdose, severe substance use and housing instability.
- Develop safety plans and make reports to MCFD or other authorities where required by law or professional obligations.
- Maintain accurate and confidential participant records in ShareVision, including assessments, case notes, service plans, referrals, and discharge documentation.
- Complete required documentation, data collection, funder reporting and other records in accordance with organizational policies, privacy legislation and funding requirements.
- Participate in team meetings, case consultations, supervision, training, and ongoing career development.
- Practice in accordance with the Code of Ethics and Standards of Practice of the BC College of Social Workers and applicable legislation.
- Perform other duties as assigned.

Working Conditions:

- Combination of office-based (New Westminster), working from home and driving/community-based work at partner sites.

Qualifications & Requirements:

- At least 1-2 years of experience working as a Registered Social Worker with at-risk youth, preferably in a trauma-informed setting.
- A successful criminal record and vulnerable sector check will be required prior to the start of employment.
- Valid BC Driver’s License and a dependable vehicle is an asset.
- Education: Bachelor's degree in social work (BSW) is required.
- Certification:



Current registration with the BC College of Social Workers is required.
- Experience with crisis intervention and case management.
- Strong interpersonal, problem-solving, and communication skills (written and verbal).
- Proficient in Microsoft Office Suite (Word, Excel, Outlook) and basic technology tools (e.g., Zoom, Teams).

About Dan’s Legacy: Dan’s Legacy provides trauma-informed and culturally sensitive counselling, outreach and job-skills training to youth at risk in Metro Vancouver. These young people, aged 15 to 25, are at significant risk of overdose, self-harm, homelessness, and suicide. Dan’s Legacy’s therapy and outreach teams go directly to the youth, our wait list times are minimal, and all our programs and services are free.

In 2025, Dan’s Legacy helped over 700 youth stabilize their mental health, and begin working towards their educational, employment and recovery goals.

Pay: $75,000.00-$78,000.00 per year
