The Nurse Practitioner position focuses on providing comprehensive care within the long-term care setting. You will work with MDs and other healthcare professionals to assess and address residents’ medical conditions while advocating for their best interests. This role involves leadership and participation in care-related discussions, ensuring that residents and families are engaged in their health journeys.
Key Responsibilities:
• Conduct assessments and manage treatment plans for residents
• Work alongside multidisciplinary teams for effective care delivery
• Lead resident care discussions during conferences
• Advocate for residents and liaise with families
• Identify trends in care to enhance team education
Requirements:
• At least one year in geriatric, rehabilitation, or long-term care nursing
• Registered with the College of Nurses of Ontario
• Leadership knowledge is beneficial
• Certifications in gerontology or mental health preferred
• Familiarity with long-term care regulations in Ontario
