Community Support Worker

$29.71 per hour
This is a non union job.
About the Role Provides and facilitates program opportunities that are conducive to the individual’s life skill development, community inclusion, and independence. Supports the individual’s vocational goals and promotes skill growth toward gainful employment if desired by the individual. Assists clients with their physical, economic, vocational, recreational, social, emotional and daily life skills development. Assists clients to achieve the greatest degree of independence and quality of life possible.

Certificate in a related Human Services, Social Services, Community Support, Disability Support, or equivalent field.
Minimum one (1) year of recent related experience.
Equivalent combinations of education, training, and experience may be considered.
Strong communication, interpersonal, and problem-solving skills.
Current First Aid/CPR and food protected certification is an asset.
Licensed homes require a negative TB test result
Must be legally able to work in Canada
Responsibilities
Develop, implement, and evaluate individualized support plans in collaboration with clients,
families, and interdisciplinary teams.
Provide person-centered support that promotes independence, life skills development, social inclusion, and community participation.
Assist with daily living activities, behavior support, medication administration, health and safety needs, and crisis response as required.
Facilitate recreational, educational, vocational, and social activities that support client goals and well-being.
Monitor client progress, maintain accurate documentation, and contribute to program planning and continuous improvement.
Collaborate with families, community agencies, professionals, and other stakeholders to coordinate services and supports; perform related duties as assigned.
