Oak Valley Health is seeking a Registered Practical Nurse to join the interprofessional team at the Reactivation Care Centre. You will assess, plan and educate patients, ensuring continuity of care during inpatient stays and discharge planning in a patient-centered setting.
You must have an active College of Nurses registration and BLS; 2–3 years in Medical/Long Term Care, with palliative care experience considered an asset. Shifts include 12-hour day and night rotations, seven days a week.
