You will play a pivotal role in an inter-skilled team, creating access for clients facing multiple barriers and complex health challenges. Your work will include patient care, community outreach, and participating in quality improvement projects. With a focus on comprehensive healthcare delivery, you will have a significant impact on diverse populations.
Key Responsibilities:
• Provide primary medical care and act on urgent lab results
• Foster collaboration with nursing and healthcare teams
• Implement and evaluate community health initiatives
• Maintain thorough documentation of client interactions
• Contribute to inter-qualified research initiatives
Requirements:
• Medical degree and valid Ontario medical license
• Membership in the College of Family Physicians of Canada
• Experience working with vulnerable populations is preferred
• Robust clinical acumen and problem-solving abilities
• Second language skills are advantageous in community outreach
