Wesley is hiring a Health and Wellness Navigator to deliver vital mental health services within the Emergency Supportive Housing Program. This full time role blends client advocacy with health support in the local community.
As a Health Navigator, you will report to the Manager of Community Housing Services and work collaboratively within a multi-disciplinary framework. Your responsibilities will include creating comprehensive service plans and providing direct support to individuals managing homelessness and substance use challenges. This role is ideal for those committed to making a real difference.
Key Responsibilities:
• Develop and implement individualized service plans
• Provide ongoing crisis management and harm reduction




• Establish trust through regular interactions with clients
• Organize harm reduction and Naloxone training
• Partner with local services for effective support
Requirements:
• Minimum of three years' experience in the field
• Relevant degree from a recognized institution
• Registration with OCSWSSW preferred
• Mental Health First Aid and CPR certifications beneficial
• Valid Driver's license and reliable transportation
