Key Responsibilities:
• Assess resident conditions and document care plans
• Collaborate in care plan implementation and evaluations

Requirements:
• Valid LPN registration
• Experience in geriatric or long-term care
• Excellent time management and communication skills
• CPR and First Aid certifications
