Key Responsibilities:
• Interview clients to assess their needs and develop service plans
• Provide comprehensive case management and follow-ups




• Advocate for clients to access necessary services
• Participate in team case reviews to improve client outcomes
• Maintain accurate documentation of client interactions

Requirements:
• Associate's degree in Behavioral Health or similar field
• Minimum of two years relevant experience
• Must obtain an Agency Affiliated license within 90 days
• Solid interpersonal and organizational abilities
• Current TB test and valid driver’s license
