The AMANI program at Windsor Essex Community Health Centre seeks a confident Case Worker to join their team. This position focuses on youth aged 12 to 29 facing co-morbid mental illness and substance use challenges. You will provide tailored, culturally responsive support that reflects the resiliency and strength of Black communities.
Key Responsibilities:
• Implement client-centered case management strategies
• Monitor and evaluate program effectiveness
• Document client progress and collaborate in case conferences
• Advocate for clients with external community resources
• Ensure culturally responsive care in all interactions
Requirements:
• Experience in counseling or therapy in community settings
• Background working with Black youth
• MSW or BSW with relevant clinical experience
• Solid communication and interpersonal skills
• Valid driver's license for transportation needs
