In this role, you will conduct home visits as a Registered Nurse or Registered Practical Nurse, addressing the care needs of a diverse patient population. Key Responsibilities:
• Evaluate patients’ needs with appropriate assessment techniques
• Develop and implement personalized care plans
• Engage with patients to build supportive relationships
• Document patient encounters and care plans thoroughly
• Be available for on-call coverage as needed

Requirements:
• Must be a Registered Nurse or RPN with CNO registration
• Driver's license and vehicle required for travel
• Prior experience in community care advantageous
• Must show reliability and flexibility
• Solid commitment to enhancing community health
