Join Jean Tweed Centre as an Aftercare Counsellor, providing essential support to women post-treatment in a hybrid environment. Deliver evidence-informed aftercare groups and follow-up services to foster recovery and connection.

In this full time, contract role, you will facilitate aftercare groups like SMART Recovery and DBT while offering ongoing support for up to six months post-discharge. Address isolation and potential relapse risks by connecting clients to vital community resources. Your work will be crucial in maintaining a pathway to recovery and promoting integrated care among various providers.

Key Responsibilities:
• Facilitate evidence-informed aftercare groups for clients
• Provide follow-up support through calls and check-ins
• Make referrals to community resources and ensure seamless care




• Maintain documentation of client interactions
• Collaborate with internal teams for coordinated care

Requirements:
• Master’s degree in Social Work, Psychology, or Counselling
• Registration with relevant regulatory body
• 3 years of experience in substance use and trauma-informed practice
• Group facilitation and program development skills
• Knowledge of Toronto community resources
