Conduct wellness assessments and monitor resident health changes Support medication management and perform MAR/TAR audits Communicate with families, physicians and care teams Monitor incidents, falls and follow-up care Maintain documentation and compliance with regulations Support infection control and clinical quality initiatives Train and support care team members What You’ll Bring:
