The Opportunity
POSITION: Community Programs Support Coordinator – ALC Transitions- Social Prescribing
CLASSIFICATION: Full-Time (37.5 hours per week), Fixed Term Ending March 31, 2027
ESCC is seeking a Community Programs Support Coordinator to provide day-to-day administrative and communications support for the Alternate Levels of Care (ALC) and Social Prescribing Programs. This position will require a highly organized, detail-oriented individual who is excited by supporting community-based initiatives and motivated by building relationships that contribute to program success.
The Community Programs Support Coordinator works closely with the ALC Project Team, the Hello Seniors team, program leadership, service partners, and others to ensure accurate data collection and reporting needs are met for the evolving programs. Reporting to the Coordinated Programs & Social Prescribing Manager, this role is ideal for an individual with strong communications skills who thrives with a varied workload in a supportive workplace.
Key Responsibilities
Program & Administrative Support - 30%
- Provide day-to-day administrative and coordination support, including but not limited to:
- scheduling meetings;
- preparing materials;
- maintaining records;
- tracking action items; and
- supporting program timelines and deliverables.
- Maintain program tools, templates, resources, files, calendars, and other administrative systems, to support the programs.
- Provide logistical meeting support such as ensuring resources and tools are accessible for attendees.
- Support the Coordinated Programs & Social Prescribing Manager in facilitation or meeting needs as required.
Communications & Partner Support – 30%
- Draft, edit, proofread and coordinate program communications, information materials, presentations, reports, partner updates, and other program related documentation or materials ensuring messaging is clear, consistent, and accessible.
- Support day-to-day communication and follow-up with community partners and service providers, including information sharing, meetings, and engagement activities.
- Maintain current program information, referral resources, forms, partner information, and community resource lists.
- Work with the Communications Manager to develop communications materials which may include, but are not limited to:
- Posters;
- Workflow diagrams;
- Presentation materials; and
- Other program related materials.
Data Evaluation & Reporting – 20%
- Support the collection, organization and tracking of program data.
- Maintain databases, spreadsheets, and tracking tools; identify missing or inconsistent information; and prepare basic data summaries, tables, charts, and other materials.
- Support evaluation activities such as surveys, outcome measures, feedback collection, and gathering client and partner stories.
- Support preparation of internal, partner, and funder reports by gathering information, compiling data and stories, drafting sections, and editing, formatting, and proofreading written materials.
- Liaise with the external evaluation contractor, program partners, and others to provide information and feedback on the program.
- Support program evaluation and continuous improvement through:
- The development and refinement of program processes, tools, templates, and resources;
- Contribute to documenting program activities, outcomes, challenges, successes, and lessons learned throughout the programs; and
- Identify opportunities, emerging issues, and gaps to improve program workflows and administration and support implementing solutions.
Short-Term Funded Supports – 15%
- Maintain accurate records and tracking of short-term funded supports, including requests, approvals, dates, amounts, utilization, and client reach for meal support and transportation as well as other required tracking.
- Assist with developing and refining tracking tools and processes related to short-term funded supports.
Organizational Supports – 5%
- Actively engage in ESCC team building activities and/or initiatives that support the growth and development of ESCC.
- Represent ESCC at community events as needed.
- Other activities as assigned.
Education and Experience
- Post-secondary diploma or degree in sociology, public health, psychology, communications, administration, social services, or a related field.
- A minimum of one to two (1-2) years’ experience in program coordination, administration, communications, data support, or a related role.
- Experience working with spreadsheets, databases, data tracking, or reporting systems.
- Experience with program evaluation, report writing, or grant-funded programs is an asset.
- Experience working with community organizations, health or social services, seniors, or multiple stakeholders is an asset.
Skills and Knowledge
- Highly organized with strong attention to detail and follow through.
- Excellent written and verbal communication skills.
- Strong organizational and time-management skills with the ability to manage multiple priorities and deadlines.
- Working knowledge of Microsoft Office 365 and relevant applications including Outlook, Teams, SharePoint, Excel, PowerPoint and other applications.
- Proficiency in various software applications, such as Canva, Adobe Photoshop and others is an asset.
- Demonstrated ability to work within complex, multi-stakeholder environments.
- Knowledge of seniors’ services, health systems, or community-based social supports is an asset.

What Does Success Look Like? Success will be demonstrated through the Community Programs Support Coordinator's ability to:
- Keep day-to-day program activities, documentation, communications, and administrative processes organized and moving forward.
- Provide accurate and timely data, tracking, and reporting support.
- Produce clear, professional communications and written materials.
- Maintain reliable tracking and documentation of short-term funded supports.
- Support strong relationships and communication with program partners.
- Identify practical improvements and contribute to the ongoing development of the ALC pilot and Social Prescribing Programs.
Why Work With Us? Salary: $58,000-$65,000 annually, commensurate with experience
Hours of Work: Full-Time, Monday through Friday (37.5 hours per week)
Term: This position is a fixed term position ending March 31, 2027 (Possibility of extension based on project results and funding commitments.)
