The Patient Navigator supports individuals and families from the Piikani Nation in accessing comprehensive healthcare services across Alberta. By bridging the gap between community-based health practices, Primary Care Networks, Mental Health Services, and the broader healthcare delivery system, this role seeks to significantly improve community members’ health outcomes.
The Patient
Navigator plays a vital role in empowering and guiding community members through the complexities of the healthcare landscape.
Role & Responsibilities
Daily Work Activities Include:
- Client Support & Advocacy: Assist Piikani Nation Members in overcoming barriers to accessing primary healthcare services across multiple providers.
- Support patient advocacy to help Piiakni Nation Members gain access to necessary resources.
- Assist and guide clients of Aakom Kiyii Health Services to ensure they receive proper support and viable solutions.
- Connect members who are unattached to primary care with available family doctors, social service agencies, mental health services, and specialty health services (e.g., physiotherapy, psychiatry, gynecology).
- Care Coordination & Logistics: Collaborate with appropriate team members to deliver comprehensive care.
- Organize and coordinate access to care services and connections within the patient’s circle of care,
including logistical support when needed.
- Help patients overcome logistical obstacles by offering transportation support and ensuring reliable access to appointments.
- Conduct home visits as required.
- Community Connection & Cultural Safety: Build robust connections with and between patients and service providers to improve access to primary healthcare and overall continuity of care.
- Support the provision of culturally specific and community-centered assistance for Indigenous persons navigating the healthcare system.
- Bring a deep respect for and understanding of Piikani Nation culture, traditions, and protocols to everyday practice.
- Follow-Up & Administration: Follow up with clients regularly to monitor progress and address ongoing needs.
- Provide educational resources and tools to empower clients toward self-sufficiency.
- Maintain accurate, up-to-date client records and complete required reports and documentation.
- Utilize electronic documentation and record-keeping systems efficiently, alongside managing supply requisitions and workload statistics.
Qualifications
- Education:
- Post-Secondary Diploma/Degree in Health Science, Social work, Psychology or related field
- Two (2) years of related experience
- Current First Aid/CPR is an asset
- Valid Drivers Licence
