Position Vacancy: PERITONEAL DIALYSIS – RENAL PROGRAM

Position: Registered Nurse – Hemodialysis

Position Type: Part Time

Position Summary:

The Home Peritoneal Dialysis Program is engaged in caring for patients that have selected home peritoneal dialysis as a renal replacement therapy. The successful applicant will work in collaboration with the Nephrologists, the Allied Health team and Nursing team to support the coordination of education, training and monitoring of patient. The successful candidate will incorporate the attitude, values, knowledge and skills required to support clinical needs and independence; introducing the concept of self-management, self-care and promoting a culture of “Home”. The successful candidate will also support peritoneal dialysis for the admitted patients

Position Requirements:

- Current Registration in good standing with the College of Nurses of Ontario
- BScN preferred
- Canadian Nursing Association (CNA) Nephrology Nursing Certification required; if not already obtained,



proof of CNA Nephrology Certification enrollment is required at the time of application and proof of successful completion is required within 6 months of hire/transfer date
- Minimum of 3 years current renal experience required.
- Peritoneal Dialysis experience preferred
- Skills in assessing, planning, implementing and evaluating the ongoing clinical management of Peritoneal Dialysis Therapy
- Demonstrated experience, ability and knowledge in the principles of adult learning and teaching
- Demonstrated ability to effectively work autonomously and within team workplace
- Computer skills and Renal Insight/Cerner/Meditech experience preferred
- Demonstrated excellent interpersonal and communication skills with an ability to work with a multidisciplinary team, including other health team members, patients, family members and visitors
- Ability to monitor, prioritize and support patient care needs in the home setting.
- Ability to be flexible and effectively work in a busy environment with changing priorities and tight deadlines
- Demonstrated sound judgment, good observation and patient assessment skills, tact and empathy
- Demonstrated evidence of recent and relevant continuing education considered an asset
- Demonstrated understanding of and commitment to Grand River Hospital’s comprehensive safety programs and practices, including staff, patient, and environmental safety, and participation in regular in-service education in this area
- Working in the community home setting is required.
- Must have own reliable vehicle and a valid driver’s license for home visits and on-call.




Demonstrated good attendance record.
