The Espanola & Area Family Health Team is an
interdisciplinary group of allied health
professionals promoting improved health of
individuals in and around Espanola by providing
increased access to a range of health care
services.
The successful candidate will have familiarity
with the health issues of the community and will
work collaboratively with other primary care
providers to improve the health of the patients’
accessing the services through the Family Health
Team.
This will be accomplished through active
employee involvement, continuous training, and
development, as well as a commitment to
compliance with policies, procedures, and
standards.

Responsibilities:
- Provide primary care to patients in keeping
with the standards of the College of
Nurses of Ontario to the full scope of
practice of the RN (Extended Class)
- In collaboration with other team members
develop and implement health education
and chronic disease management
programs
- Provide health teachings and support to
patients
- Work collaboratively with other FHT staff
and community organizations to improve
patient care, service delivery and
coordination
Qualifications:
- Baccalaureate or master’s in nursing, NP
Certificate from an accredited program
- Current Extended Class registration in
good standing with the College of Nurses
of Ontario
- Demonstrated experience working in
community setting or primary care setting
- Strong clinic skills – excellent health
assessment skills and diagnostic skills
- Ability to order/interpret results of
screening, diagnostic lab tests, x-rays and
ultrasounds
- Demonstrated ability to work effectively
within an interdisciplinary team
- Strong interpersonal and communication
skills
- Fluency in English and French is an asset

pension plan. Starting wage is $59.931 –
$67.669 per hour based on experience.
- Up to $35,000 in recruitment incentives
based on eligibility.
