Nurse Practitioner (NP) – Primary Care

Ajax Family Practice & Walk-In Clinic is seeking a compassionate and clinically confident Nurse Practitioner (NP) to join our team. The role includes a balance of same-day/walk-in care, routine primary care, follow-ups, preventive health, and chronic disease management for patients across the lifespan.

With an established patient population, consistent patient flow, and experienced family physicians readily available for collaboration, you can focus on providing thoughtful, high-quality care.

The Role

- Assess, diagnose, treat, and manage patients across the lifespan
- Provide same-day and walk-in care for common acute and episodic concerns
- Manage routine primary care, follow-ups, preventive care, and chronic disease needs
- Order and interpret investigations and prescribe within NP scope
- Provide patient education, counseling, and appropriate follow-up
- Coordinate referrals and collaborate with physicians when appropriate
- Maintain accurate and timely documentation using OSCAR EMR

Qualifications

- Current Nurse Practitioner (RN-EC) registration in good standing with the College of Nurses of Ontario
- Appropriate NP education and certification for primary healthcare practice in Ontario
- Strong assessment, diagnostic, prescribing, and clinical decision-making skills
- Excellent communication and patient-care skills
- Primary care, family medicine, community health, urgent care, or walk-in experience preferred
- OSCAR/EMR experience is an asset
