St. Joseph’s Health Centre is seeking a Mental Health & Dementia Coordinator to lead the development, implementation, and sustainability of mental health and dementia programming across our Long-Term Care homes.

Reporting to the Senior Administrator, the Mental Health & Dementia Coordinator provides program leadership, clinical expertise, and operational support to promote the emotional, mental, and spiritual well-being of residents within our compassionate, person-centred model of care.

Working collaboratively with Directors of Care, leaders, physicians, healthcare professionals, residents, families, and community partners, the Coordinator supports best practices, education, quality improvement, and consistent approaches to mental health and dementia care across the organization.

Key Responsibilities

- Lead the ongoing development, implementation, evaluation, and sustainability of mental health and dementia programs across SJHC Long-Term Care homes.
- Develop and maintain systems, processes, policies, procedures, and quality standards that support evidence-informed and person-centred care.
- Act as a subject-matter expert and resource to Directors of Care, managers, physicians, and interdisciplinary healthcare teams on mental health and dementia-related matters.
- Provide functional and operational support to Long-Term Care leadership regarding mental health and dementia programming.
- Provide informal clinical leadership, coaching, mentoring, and education to staff and interdisciplinary teams.
- Lead mental health and dementia education, awareness, capacity-building, and skill-development initiatives.




- Supervise site-based Mental Health Clinicians, including recruitment, onboarding, coaching, performance management, attendance support, conflict resolution, performance evaluation, and progressive discipline.
- Identify opportunities for quality improvement, risk mitigation, and enhanced clinical and operational performance, and support the implementation of improvement initiatives.
- Participate in strategic planning and develop short- and long-term program goals, plans, and recommendations.
- Develop and maintain collaborative relationships with community partners, healthcare providers, students, volunteers, residents, and families.
- Coordinate care and services across the continuum to support residents with complex mental health and dementia needs.
- Monitor program effectiveness and identify opportunities to enhance resident outcomes and the overall resident experience.
- Travel between SJHC Long-Term Care sites to provide program leadership, support, monitoring, and development.
- Ensure programs and practices comply with the Fixing Long-Term Care Homes Act, 2021, applicable regulations, and other relevant legislation, standards, and organizational policies.
- Assist with mandatory reporting processes and required documentation, including High-Intensity Needs reporting.
- Remain current with emerging research, trends, legislation, and best practices related to mental health and dementia care.

Qualifications





- Bachelor’s degree in a health-related field.
- Minimum 1–2 years of experience working with individuals and families within the mental health and/or dementia system.
- Experience working in Long-Term Care is considered an asset.
- Demonstrated knowledge of mental health and dementia care practices, legislation, and related healthcare systems.
- Knowledge of the Fixing Long-Term Care Homes Act, 2021, Occupational Health and Safety Act, Human Rights Code, privacy legislation, and applicable mental health legislation is considered an asset.
- Demonstrated critical thinking, analytical decision-making, problem-solving, and risk assessment skills.
- Demonstrated ability to teach, coach, mentor, and build capacity within individuals and teams.
- Excellent interpersonal, communication, organizational, planning, and time-management skills.
- Demonstrated ability to work collaboratively with interdisciplinary teams and external partners.
- Proficiency with Microsoft Office applications, including Word, Excel, and PowerPoint.
- Bilingualism in Canada's official languages is considered an asset.
