We are seeking compassionate Community Social Workers to support individuals and families by understanding their social needs and connecting them with appropriate community resources and services. The role involves community outreach, one-on-one support, home or community visits, information sessions, and referrals to help people access available assistance.

Community Social Workers will speak with individuals about challenges related to housing, employment, education, family support, financial assistance, and community services. They will provide basic guidance, help individuals identify suitable resources, and connect them with relevant organizations and support programs.

Key Responsibilities:

- Conduct community outreach and engage with individuals and families.
- Identify basic social and community needs through one-on-one interactions.
- Provide information about available community programs and support services.
- Connect individuals with housing, employment, education, and social support resources.




- Assist community members in understanding available services and programs.
- Conduct home, community, or outreach visits when required.
- Organize community meetings, information sessions, and support activities.
- Maintain basic records of client interactions, referrals, and outreach activities.
- Work with community organizations and other social service workers.
- Maintain confidentiality, respect, and professionalism when supporting individuals.
