Title: Patient Navigator Division: Clinical Services Affiliation: Public Service Alliance of Canada Site: Moose Factory, ON Reports to: Clinical Manager Status: Permanent Full-Time The Weeneebayko Area Health Authorityis seeking an experienced professional to fill the key role of Patient Navigator. Reporting to the Clinical Manager, you will assist patients in navigating the WAHA, provincial, and federal health systems.

- Act as a resource and facilitator to discharge planning team, physicians, and staff.
- Assist in the development of care plans when clients are back in their home settings.
- Monitor clients progress while they are back in their home settings to determine if needs change over time.
