We are looking for enthusiastic and motivated Community Nurses to support our NHS clients in Oxford. We are offering long term contracts as well as Ad-Hoc shift work as well as competitive rates of pay and mileage.
Responsibilities - Coordinating care for individuals with long-term conditions or complex health needs - Administering medication and supporting self-administration - Dressing and managing wounds - Providing Catheter Care in Clinics and within the individual’s home - Providing Wound Care for housebound individuals after surgery - Supporting issues around continence - Cannulation / IV Therapy administration - Caring at end of life Experience - 6 months experience of working at relevant level within the community in the UK - Has a full UK driving license and access to an insured car daily - Able to work within a diverse community of patients and health care professionals.
- Effective communication skills written,
verbal and non-verbal with the ability to adapt communication for individuals with communication difficulties - Able to make clinically reasoned judgements when working with a variety of conditions.
- Able to work autonomously, and ability to know when to seek appropriate advice / guidance from senior staff.
- Effective time management and caseload management.
- Positive problem-solving skills.
- Working knowledge of clinical outcomes measures, clinical audit.
- Experience with EMIS, undertaking assessments, providing holistic care to elderly patients on DN caseload - Management of long-term conditions and palliative care - Aware of safeguarding adults - Search for health needs and working with the core and wider MDT to meet patient needs
