What would my role be? Reporting to the Director, Family Health Services, you will:
- Work collaboratively with members of the health care team in providing healthcare for family medicine patients
- Provide specialized care to an identified group of patients who are at risk
- Participate in committees which address the needs of at-risk patients
- Provide education to learners and staff.
How do I qualify?
Required
- Undergraduate degree in Nursing, Master’s degree for graduates after 2011
- Valid certificate of registration from the College of Nurses of Ontario (CNO) as NP-PHC
- Current membership in Nurse Practitioners’ Association of Ontario (NPAO)
- Primary Health Care Nurse Practitioner Certificate.
- Professional Liability insurance with Canadian Nurses Protective Society
- Valid Basic Life Support (BLS) cardiopulmonary resuscitation (CPR) Level C certificate
- Recent related clinical experience (work or educational related)
Preferred:
- Fluent in French and English
What skills should I possess to succeed in the role?
- Excellent patient assessment, critical thinking, clinical reasoning, communication (oral and written), and problem-solving skills.
- Practice autonomously according to the NP standards of practice
- Able to work in an inter-professional team environment.
- Able to prioritize, manage time effectively and be flexible in a very active work setting.
- Intermediate computer skills, including in electronic health records
- Advanced knowledge of medical terminology
