AgeCare is seeking a Nurse Practitioner to collaboratively deliver resident care at AgeCare Queen’s Garden in Hamilton, ON. You will work with physicians and the interdisciplinary team to assess, diagnose, and manage acute and chronic conditions while following regulatory standards. Responsibilities include care conferences, care leadership for the LTC team, and engaging with residents, families, and SDMs. Prior experience in geriatric or long-term care and relevant certifications are valued.
