In this position, the System Navigator will provide vital case management and assistance to individuals living with HIV, helping them navigate complex healthcare systems. Key Responsibilities:
Coordinate client services across healthcare systems
Offer direct referrals for HIV treatment options




Conduct regular reviews of client goals and progress
Assist clients with understanding community resources

Requirements:
Degree in a relevant field, such as social services
At least 2 years in direct client support
Valid G Ontario Driver’s License; access to a vehicle
Recent vulnerable sector check required
