Collaborating with Care Coordinators and clinicians, you will implement evidence-based practices to ensure successful outcomes. Your role involves direct engagement, policy support, and outreach to enhance care standards. Key Responsibilities:
Conduct clinical assessments and develop care plans
Act as a resource for interdisciplinary team members
Apply best practices and monitor treatment effectiveness
Assist with supply management and recommendations
Engage in data analysis for program improvement Requirements:
Registered Nurse with a current license in Ontario
Certified in NSWOC
At least 5 years of nursing experience
Understanding of relevant healthcare laws and policies
