Key Responsibilities:
• Accurately enter medication orders in Meditech
• Conduct clinical rounds with healthcare professionals




• Review patient medication profiles critically
• Assist prescribers with medication-related questions
• Monitor and report adverse drug reactions

Requirements:
• Degree in Pharmacy or Doctorate in Pharmacy
• Valuable standing with the Ontario College of Pharmacists
• Hospital pharmacy experience preferred
• Detail-oriented with strong problem-solving skills
• Excellent written and verbal communication skills
