As a Registered Practical Nurse reporting to the Director of Care or Health and Wellness, you will deliver planned, patient-centered care according to provincial standards. Key duties include performing clinical assessments, documenting care plans, and administering medications while upholding Extendicare’s standards. Solid communication and teamwork skills are essential in this role, especially when addressing resident needs and supporting their families.

Key Responsibilities:
• Work within a collaborative interprofessional team
• Conduct clinical assessments and maintain accurate documentation




• Implement and evaluate care/service plans as needed
• Provide direct care and administer medications to residents
• Facilitate resident and family education sessions

Requirements:
• Current registration as a Registered Practical Nurse
• Experience in senior care and rehabilitation preferred
• Solid critical thinking and communication skills
• Basic Cardiac Life Support certification required
• Computer proficiency with Microsoft Office applications
