Flemingdon Health Centre seeks a Community Outreach Service Worker for Chronic Diseases and Mental Health. This part-time position involves extensive community engagement and program support.

As a part time, 6-month contract worker, you'll enhance chronic disease management through outreach, education, and cooperative efforts. You will work with diverse populations to inform them about diabetes and other health services, while contributing to quality improvement initiatives and evaluations. Evening and weekend hours will be required due to community events.

Key Responsibilities:
• Network with leaders on chronic disease prevention
• Promote education programs via various channels
• Support health education session delivery
• Organize and analyze health-related data
• Collaborate with colleagues to improve service delivery

Requirements:
• Relevant university education or experience
• Demonstrated skills in community outreach
• Team-oriented with robust interpersonal skills
• Proficient in MS Office and Canva programs
• Strong initiative and project management capabilities
