AgeCare is recruiting a Nurse Practitioner to join Generations Toronto, a not-for-profit long-term care campus delivering resident-centered care in a 122-bed home. The role works collaboratively with physicians, the director of care, and interdisciplinary teams to meet individual resident needs.
You will assess, diagnose, and manage acute and chronic conditions, provide secure treatments, and lead clinical care planning, including mobility, palliative care, and family communication.
