As a Full time Follow Up Worker, you will assist clients with housing assessments and develop personalized plans to support their transition out of homelessness. Additionally, you’ll provide life skills training and advocate for clients within various support systems to ensure comprehensive assistance and long-term success. Key Responsibilities:
Conduct initial assessments with clients for housing options
Develop individualized housing and service plans
