Role Description The Brain Injury Support Facilitator is a full-time, on-site role based in Okanagan, BC. This position involves providing direct support to individuals with acquired brain injury through one-to-one sessions, group programs, and community-based activities. The facilitator will help clients set and work toward personal goals, develop life skills, enhance independence, and connect with community resources and services.

Day-to-day responsibilities include developing support plans, documenting progress, collaborating with interdisciplinary partners and families, and advocating for clients’ needs. The role also includes participation in team meetings, contribution to program development,



and adherence to organizational policies and standards of care.

Qualifications

- Experience providing direct support or case management to individuals with disabilities, mental health conditions, or acquired brain injury.
- Strong skills in client assessment, goal setting, and individualized support planning.
- Effective communication, active listening, and interpersonal skills, with the ability to build trust and rapport.
- Organizational and time management skills, including accurate documentation and record-keeping.
- Ability to work collaboratively in a multidisciplinary team and maintain professional boundaries.
- Knowledge of community resources, social services, and advocacy practices relevant to brain injury support.
- Post-secondary education in social work, psychology, rehabilitation, human services, or a related field (or equivalent combination of education and experience).
- Comfort supporting clients in community settings, with a valid driver’s license and reliable transportation considered an asset.
- Commitment to trauma-informed, person-centered, and culturally secure practices.
- Clear criminal record check and any other screenings required for working with vulnerable populations.
