Role Description This part-time, remote Physical Therapist role focuses on providing evidence-based therapy and rehabilitation services to patients in a home-based or virtual setting.

The Physical

Therapist will assess patient conditions, develop individualized treatment plans, and guide patients through therapeutic exercises and interventions to restore function, reduce pain, and promote mobility. Day-to-day responsibilities include conducting virtual or telehealth evaluations, documenting clinical findings and progress, educating patients and caregivers on home exercise programs, and collaborating with other healthcare professionals to coordinate care. The role also involves monitoring patient outcomes,



adjusting treatment plans as needed, and ensuring that all care is delivered in alignment with professional standards and regulatory requirements.

Qualifications

- Candidates should possess strong clinical skills in Physical Therapy and Rehabilitation, with the ability to design and implement individualized treatment plans.
- Candidates should possess a solid foundation in Medicine and Patient Care, including safe, evidence-based practices and interdisciplinary collaboration.
- Candidates should possess experience in Home Care and remote service delivery, with comfort using telehealth platforms and digital documentation tools.
- Active state licensure (or eligibility for licensure) as a Physical Therapist and completion of an accredited Physical Therapy program (DPT or equivalent).
- Robust communication, time management, and organizational skills, with the ability to work independently in a remote, part-time setting.
- Experience working with diverse patient populations and a commitment to equitable, inclusive, and person-centered care.
- Familiarity with electronic health records (EHR), outcome measures, and quality improvement processes is beneficial.
