: Under the direction of the Manager of the Transplant Program, the Social Worker supports the development and delivery of the transplant program as an integral member of the health care team. The incumbent assesses clients and their families and provides counseling and psychosocial support to individuals coping with the complexities of organ failure and transplantation.

The Social

Worker provides ongoing support throughout the pre- and post-transplant phases, helping clients and families navigate the emotional, social, financial, and practical challenges associated with transplantation. The incumbent collaborates closely with members of the interdisciplinary transplant team to promote continuity of care, facilitate access to appropriate resources and services, and support positive outcomes for clients and their families.

The Social

Worker functions as an active member of the transplant program, with an emphasis on a collaborative, client-centered, and team-based approach to care. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

- Bachelor of Social Work (BSW)

- Ability to work as a member of a multi-disciplinary team

- Knowledge in developing policies, procedures and standards
- Ability to develop a monitoring program to assess and improve quality of care
