The Saskatchewan Health Authority in Watrous is seeking an Assessor Coordinator for Home Care to assess and coordinate client services and case management in the Watrous District Health Complex. You will empower clients and families through client-centered planning, manage referrals to other agencies, and ensure confidentiality and HIPA compliance within a multidisciplinary team. The role requires a degree in health or social science, and experience in palliative care is an asset.
