Lakeland Care, Inc. in Appleton, WI, is seeking a part time Care Manager to join an interdisciplinary team serving frail elders and adults with disabilities. The role covers care management, service coordination and member-centered planning, with travel up to 2 hours from the Appleton area. You will assess outcomes, coordinate with the RN Care Manager, monitor plans, support access to programs, and maintain DHS-compliant records while protecting PHI and member rights.
