Are you passionate about helping seniors navigate their health journey with confidence and compassion? As a Health Navigator at Chartwell, you'll work closely with the Health & Wellness Manager to coordinate resident assessments, care plans, and wellness services. You'll collaborate with residents, families, physicians, and the care team to ensure every resident receives personalized, high-quality care in a welcoming retirement community.

What You'll Do
Complete resident assessments and develop individualized care and service plans
Coordinate resident admissions, care conferences, transfers, and discharges
Provide nursing care, medication administration, and resident education as required
Partner with physicians, families, and the care team to support changing resident needs
Maintain accurate clinical documentation and monitor resident wellness outcomes
Support infection prevention, quality improvement, and regulatory compliance initiatives

What You'll Bring
Registered Practical Nurse (RPN/LPN) or Registered Nurse (RN) in good standing with the provincial College of Nurses




Current CPR/First Aid certification and medication administration knowledge
Experience in retirement living, long-term care, assisted living, or a related healthcare setting
Strong assessment, clinical, organizational, and communication skills
Compassionate, collaborative, and resident-focused approach to care
Experience working with electronic health records and Microsoft Office

Additional Requirements
Ability to meet the physical demands of resident care
Flexibility to respond to emergencies and participate in Manager-on-Duty or on-call rotations, where required

Explicit Criminal Background Check, VSS and TB Test where required
