• Support residents during their team-oriented living journey • Facilitate essential family advocacy during transitions • Enhance overall well-being and respect residents’ individuality • Coordinate and lead care conferences for effective communication • Identify community services to support long-term care

Requirements: • Two-year diploma in Social Service Work is essential • Registration with the Ontario College of Social Workers required • Relevant experience with seniors and palliative care • Bilingual skills in French and English are a must • Knowledge of medical terminology and community resources
