SOAHAC is hiring a full-time Diabetes Nurse Educator in London, Ontario. You will provide culturally secure, client-centered education and support to Indigenous adults with Type 2 diabetes and pre-diabetes.
Join a multidisciplinary team to develop care plans, lead group education, and participate in program monitoring to meet ministry requirements. Flexibility for local travel and a strong commitment to community health is expected.
