You’ll engage with clients through comprehensive in-home assessments, individualized treatment plans, and recommend essential equipment modifications. You'll also collaborate closely with an interdisciplinary team, all while maintaining your professional flexibility and independence.

Key Responsibilities:
• Conduct in-home assessments including functional evaluations
• Develop individualized, evidence-based treatment plans
• Provide follow-up visits focused on goal progression




• Recommend assistive devices and home modifications
• Educate clients and families on successful aging in place

Requirements:
• Registered and in good standing with COTO
• Experience in home and community care
• Strong skills in functional assessments and safety evaluations
• Valid driver’s license and reliable transportation
• Excellent documentation skills in clinical settings
