Lead integrated health services as the Integrated Care Manager at Durham Community Health Centre. This full-time role emphasizes team coordination, quality improvement, and collaboration across diverse programs and services.

As a key member of the management team, you will report to the Vice President of Integrated Care and Experience. The Manager ensures delivery of high-quality, client-centered services while supervising interprofessional staff and promoting a culture of collaboration. Your leadership will enhance program integration and support the strategic objectives of the Durham Community Health Centre.

Key Responsibilities:
• Implement strategic and operational plans aligned with DCHC goals
• Supervise interprofessional teams, championing collaboration and accountability




• Monitor program performance against financial and operational targets
• Foster internal referrals for coordinated client-centered care
• Ensure compliance with DCHC standards and funder requirements

Requirements:
• Undergraduate degree in health care or related field
• Minimum 5 years in community-based health organizations
• Experience in integrated care delivery and performance management
• Solid knowledge of health equity and determinants of health
• Excellent communication and organizational skills
